It could be a number of causes. I am in no way able to give medical advice, but my understanding is if there is frank blood in the urine, red adn fresh and you can actually see it, it can be from the bladder or any of the tubes connecting from the kidney to the outside world.
Infection, trauma from rough handling (usually in adolescent boys), sporting injury, benign polyps etc.
If its from the kidneys, quite often the blood is 'filtered' for want of a better word so it is often not visible very well to the eye. However if it is a kidney stone or soemthing like that, and it is moving around, it woudl cause pain and bleeding. Kidney stones can be small like gravelly sand, or quite large in one 'stone' arrangement and have 'spikes' on them.
They can be extremely painful.
In my experience, and I have been wrong many times, for frank blood to be a system or major health problem, his BP would/could be high, or he has sustained a major heat stroke or recent major organ/muscle trauma for the extra protein to be leaking through his kidneys from the blood stream.
It is very good he has gone for help for sure. I am sure you will get it sorted out soon. Keep in touch with your medical people
The ex A had many health issues. The issue for me was his health problems always took precedent over mine. I ended up in the hospital twice. Both times he down played them. One time I was in the hospital for a week.
I have had to really work on taking care of me irregardless of how traumatic, self destructive and all consuming other people's health issues are.
